Animal Management Application Forms
Animal Registration: Renewal or Change of Details	Application for registration/ Renewal of registration/ Change of details of registration of cat or dog
Animal Surrender Form	For Mount Isa City Council to collect your animal, the Animal Surrender Form needs to be completed and submitted to Council.
Breeding Dogs or Cats	Application for Authorisation/ Change of details for breeding dogs or cats
Keeping more than 2 dogs or cats	Application for Authorisation/Change of details of authorisation for keeping more than 2 dogs or cats

Council Property Alteration or Hire Application Forms
Temporary Use of Footpath and Public Places	Application to partially obstruct a footpath or public place, commonly in the CBD area, for purposes including: mobile ticket selling, stalls, raffles etc
Temporary Road Closure and/or Special Event	Application to temporarily close a section of a public work and/or to conduct a special event adjacent to a road. For example: Construction works, fair or auction or street procession.
Temporary or Permanent Works on Council Property	Applies to anyone working on Council property, commonly the footpath. Works may include: new driveways (residential and commercial), connecting to council storm water drainage and permanent landscaping.
Hire of Grandstands	Application to hire a Council Grandstand for an event
CBD Banner Display	Application to display banners on the CBD Banner poles as indicated on the map included in this form
Use or Hire Park	This will notify Council crew to avoid performing works during an event time at a specific park and also to ensure the park is booked for your event.
Transport a Building	Application to transport a building within the Mount Isa Shire.

General
Customer Request for Service	For requests on Council’s services. Requests will be attended to within 10 working days.
Damage/Claim Against Council	For damages or claims to Mount Isa City Council
Search Request	For search requests relating to: Rates and requisition Rates search with water meter reading Limited town planning certificate Building, plumbing and health search

Town Planning Application Forms
Boundary Clearance Dispensation	Application to apply for exemption to the boundary clearance standards outlined in the Town Planning Scheme. All relevant information is located in this form.
Material Change of Use (Multiple Dwellings)	Development Application Package for a Material Change of Use of a property to Multiple Dwellings
